RN-Nurse Navigator-Cancer Support Services

Job Objective: Utilizes specific clinical knowledge to offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process provides education and resources to facilitate informed decision making and timely access to quality health and psycho social care throughout all phases of the care continuum. Works with patients referred for assistance in accessing and utilizing health care resources due to their complex chronic illness.
