RN-Nurse Navigator-Ambulatory Access-Ambulatory Patient Advance Access

Job Objective: Assures that communication with each caller will be handled in a professional and tactful manner. When appropriate, the caller’s symptoms will be assessed and triaged using the nursing process and approved guidelines in order to assist in obtaining the appropriate level of care and/or self-care advice. This position will also assist and be a resource for all aspects of the medication refill, prior authorization process and patient navigation through the health system. This role will communicate and work effectively with patients and various members of the healthcare team including clinical support staff and providers.
