# **Behavioral Health Counselor - Brook Lane** Brook Lane is seeking a compassionate and motivated **LMSW, LGPC, LCSW‑C, or LCPC** to join our Cumberland, MD team. You will: * **Complete psychosocial and mental health assessments** to identify patient needs. * **Diagnose, counsel, and support** patients and families through a variety of behavioral health challenges. * **Develop and implement individualized treatment plans** that promote healing, stability, and improved functioning. * **Provide education, resources, and referrals** to help patients adjust to illness and maintain optimal personal and family well‑being. * Collaborate with interdisciplinary teams to ensure coordinated, patient‑centered care.
