Job Description
**Social Worker Care Management | Full-time, 80 hours bi-weekly** **Monday-Friday, 12p-8pm** _Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge.
