Speech Language Pathologist- Home Care

Position HighlightsDepartment: Home Care Speech and Language TherapyHours:  Per VisitPosition SummaryAdministers Speech and Language Therapy services under medical prescription in accordance with Hospital regulation, State of Connecticut Practice Act, and quality standards.  
Essential Duties & ResponsibilitiesDevelops a plan of care for each patient, with the patient's input and agreement, that addresses the patient's safety, educational, psychological, and developmental needs. Interprets and assesses data about the patient, as evidenced by appropriate goal setting and goal revision throughout the plan of care, while explaining treatment goals, plans, techniques, and outcomes to the patient and/or family.Displays expertise in recommending various therapeutic procedures and modalities. Complies with professional and departmental documentation standards. Addresses a home exercise program with each patient. Provides a written home exercise program and places a copy in the patient's chart. Maintains adequate caseload, adhering to departmental productivity standards. Attends continuing education seminars as required by licensure and on return from such an event, schedules and presents an educational program, including a test, to Rehab staff or other medical professionals. Participates in new program development, physician marketing, and education of M.D's in the community about Rehab Services. Communicate regularly with the Rehabilitation Supervisor, other team members, and referring physicians for patient care. All other job-related duties as assignedMinimum QualificationsGraduate of an accredited School of Speech-Language PathologyLicensed or eligible for licensure to practice in the State of ConnecticutPreferred Skills: Experience with electronic medical charting3 years clinical experienceFlexibility with scheduling About Middlesex HealthThe Smarter Choice for your Career!Come join one of Connecticut’s Top Workplaces, and a Magnet designated organization!
