Job Description
OverviewProvides collaborative care management services in an outpatient setting, coordinating care between primary care providers and consulting psychiatrist. Tracks patients using a registry, prioritizes cases for review, and provides treatment recommendations to primary care providers. May also offer brief, evidence-based psychotherapy (e.g., problem-solving therapy, motivational enhancement therapy, behavioral activation) and psychosocial interventions to support treatment, including medication adherence, follow up care, and specialty referrals. Implements care recommendations, monitors symptoms, and serves as a liaison between providers. Bilingual (English/Spanish) skills and behavioral health care management experience preferred.
