Per DiemSummary:Social Work services at Middlesex Hospital encompass a wide scope of assistance for patients and families. The primary focus is to work directly with the patients and families to assess where they are in their process of understanding the diagnosis/prognosis and coping. We promote communication between the patient, family and health care providers. A comprehensive psychosocial assessment is completed by social work to identify needs and goals. Social work then works collaboratively with the patients and families toward achievement. Services provided by social work clinicians include but are not limited to; Responsibilities: Supportive counseling, crisis intervention, teaching regarding disease progression, advanced directives, and end of life planningKnowledge and referrals to community resources, discharge planningAccess/education regarding entitlements and assistance with completion of entitlementsFinancial stressors and insurance coverageFacilitate family meetingsActive members on an interdisciplinary teamQualifications:Hold a master’s degree from a social work program accredited by the Council on Social work Education State of Connecticut License in Clinical Social Work requiredGood analytical, organizational and interpersonal skillsMust be able to communicate effectively orally and in writing.Must be able to work collaboratively with other members of the health care teamComputer Literacy
