The Certified Nutrition and Outpatient Diabetes Education Coordinator is a healthcare professional, R.N, or R.D, who focuses on helping people who have diabetes as well as who are at risk for diabetes and related conditions achieve behavior change goals, which in turn, lead to better clinical outcomes and improved health status. In addition, is responsible for all office operations, inclusive of ensuring billing and coding compliance, denial review and corrections, tracking data pertinent to accreditation, and all other remaining office oversight responsibilities. Diabetes educators apply in-depth knowledge and skills in the biological and social sciences, communication, counseling and education to provide self-management education/ self- management training in the form of individual, classroom and community education. The Certified Nutrition and Outpatient Diabetes Education Coordinator will coordinate the daily operation of the Program as well as be a liaison for AH In-patient to Out-patient transition needs and work closely with inpatient diabetes educator, as well as participate with hospital discharge planning of diabetic patients. S/He coordinates medical office assignments in off-site locations as well as the Medical Home Clinic. **Education, Licensure/ Certification, Experience:** * Bachelor's Degree Required * Certified with AADE as a Certified Diabetes Educator or greater. * Registered by the Academy of Nutrition and Dietetics as a Registered Dietician. * Master's preferred. * Maintains professional licensure or registration as well as continuing education requirements to maintain CDE certification. * Registered Dietitian or Registered Nurse. * Certified Diabetic Educator. * Minimum 3 years' experience. **Essential Job duties:** * Essential Functions • Sets priorities, assigns responsibilities, and establishes appropriate timetables. • Monitors and evaluates the accomplishments of departmental goals and is responsible for the annual report to the American Diabetes Association (ADA). • Ensures compliance with all federal, state, and local regulations. • Assists in achieving compliance with ADA Standards, DNV standards, and other accrediting organizations with regulatory agency standards by providing services to inpatients as well as outpatients. • Directs the overall operation of the Nutrition and Diabetes Education outpatient program, including scheduling, evaluations, hiring and disciplinary functions needed. • Communicates effectively and frequently with other Diabetic Educators and staff as well as Medical Director on any changes to program as well as updating curriculum a minimum of annually. • Organizes interdepartmental and interdisciplinary meetings, task forces, and projects. • Participates in the orientation and training of new diabetic educators, as necessary. • Directs daily operations of nutrition and diabetic education program for patients including screening, assessment, interventions, menu planning, and customer satisfaction levels are met. • Utilizes adult learning strategies in program planning to select the content, teachings, strategy and method appropriate for the learner • Counsels and educates patients individually or in a group on diabetes care in conjunction with Medical Director. • Maintains patient privacy when working with patients. • Performs duties and tasks as assigned by manager. • Provides marketing of Diabetes Education and MNT program to community and possible referring MDs, including organizing the annual Diabetes Day for the community. * Business Functions • Maintains productivity standards and practices effective time management and prioritizing of tasks. • Manages resources (time, materials, and staff) in a cost-effective manner. • Ensures compliance in all billing and coding matters. • Ensures all referrals are compliant prior to scheduling of patient. • Ensures denials are worked timely, if patterns emerge from denial review, provide team education on what detail is lacking to ensure correct billing submissions moving forward. • Provides Leadership in office operations to ensure team member needs are met and goals are achieved. • Assists in developing and revising patient education materials, forms, and protocols. • Identifies opportunities to increase patient, physician, and community awareness of outpatient diabetes education services. • Assists in achieving compliance with DNV, include state and federal. • Supports and promotes the organizations' infection control, safety, risk management, and patient experience programs. • Ensures employees have appropriate equipment, supplies and resources to perform their jobs and meet goals, cost controls and deadlines. * Professional/Career Development • Assumes responsibility for own personal education and developmental needs. • Leads and coordinates regular staff meetings and in-services. • Maintains diabetic education certification and continuing education requirements. • Participation in professional organizations and activities workshops, seminars, and staff development programs. • Leads/participates in departmental and interdisciplinary meetings, task forces, and projects. * Personnel Management • Provides direction and leadership to all staff, participates in planning, implementation, and evaluating services. • Conducts performance reviews, provides counseling and mentors staff. • Assesses the delivery of services by staff within department. Identify problems and areas for improvement. • Coordinates personnel activities to include but not limited to orientation, evaluation, counseling, and staff development. • Promotes open communication with all staff. Plans and conducts regular staff meetings. • Recruits, interviews, and selects staff in collaboration with the department director as needed. • Resolves problems relating to staff and patient issues in a timely and efficient manner. * Professional Expectations • Maintains positive/professional relationships with colleagues (Augusta Health and community agencies). • Displays self in a professional manner. • Acts as a resource for all clinical and administrative staff. • Provides suggestions/opportunities to improve efficiencies for processes within own department.
