Job Description

Citizens Memorial Hospital (CMH) is seeking a Behavioral Health Navigator and Peer Specialist to work with our patients. CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!

About the department: The Care Transitions Department coordinates specialized healthcare navigation and peer support. By bridging the gap during care handoffs, the department ensures patients safely move from inpatient or crisis environments back to community-based and outpatient mental health care.

A typical day includes: This grant-funded position facilitates patient scheduling, coordinates care, refers patients to appropriate behavioral health settings, and conducts timely follow-up with patients.

Receives and processes internal/external provider and patient referrals for behavioral health services.
Acts as the bridge between clients, specialized behavioral health providers, and outside community support services.
Models resiliency, provides positive reinforcement, and helps patients identify and overcome recovery barriers.
Collaborates directly with the Mobile Integrative Health Department's Maternity Care Team 4 times a month to deliver in-home peer support services.
Ensures accurate, compliant data entry and case tracking for internal and external program reviews.

Job Requirements:

Education: High school diploma or equivalent required; Certified Peer Specialist training completion within 6 months of hire
Experience: At least 1 year of prior experience working in a behavioral health or clinical setting required
Skills: Communication, ability to maintain confidentiality, and basic knowledge of community-based resources
Licensure/Certification: Certified Peer Specialist through Missouri Department of Mental Health within 6 months of hire
Physical: Ability to sit for extended periods of time while performing computer tasks; ability to occasionally lift up to 20 lbs.
