Care Coordination Health Partner - PRN

Departmental OverviewCompensation$21.15-$34.55/hr. depending on previous work experience.The Care Coordination Health Partner (CCHP) is responsible for assisting in delivery of services that address the Social Drivers of Health, referral assistance, and bridging gap between communities, individuals and providers. The CCHP will serve as an extension of the health care team and be responsible for helping patients and their families navigate medical services and community resources to improve population health and increase patient self-sufficiency. Works closely with the Care Coordinators to meet the patient needs towards set goals including hospital follow up for inpatient/ER visits, medication compliance, coordination of services, community resources, and addressing Social Drivers of Health.Supports the Care Coordination departments in coordinating, procuring and managing resources in order to facilitate quality care, improve patient cost outcomes and assist in assuring a smooth transition home or to the next level of care.
