Our department includes Clinic Care Coordinators who work directly with Norman Regional Clinics and their patients to provide an enhanced level of health promotion, care coordination, and disease management. We enjoy a collaborative process with both clinic based staff, acute care staff, and post-acute providers to ensure continuity of care, so our patients can truly consider Norman Regional Health System their medical home. The Care Coordination Health Partner see's patients in the clinic and/or via phone conference to assist patients with health care navigation needs. Job SummaryJob SummaryCollaborates with Care Coordinators as well as Providers to assist with coordination of care with multiple external agencies to best serve the needs of the patientAct as a patient advocate and liaison between the patient, caregivers, healthcare team and community service agenciesKeep abreast of current trends in the field and integrate knowledge gained into current work practice, with support of Care Coordination leadership.Utilize motivational interviewing skills and culturally sensitive methods to collaborate with patientsMaintains high degree of communication with Care Coordinators and other clinical team members to assist in ongoing care coordination needs and communicate/remove barriersPlanning/Organizing/InitiatingParticipates and supports departmental quality improvement initiatives by sharing ideas and experiences and is willing to encourage others to do the sameProvide exemplary customer service skills both with patients, caregivers and community partners QualificationsQualificationsCustomer service experience preferredBilingual preferred but not requiredBasic computer skillsExcellent communication both oral and writtenHighly organized and self-motivatedEducationBachelor’s degree in any fieldIn lieu of degree will accept 4 years customer service experience or experience in the insurance/social work/medical fieldsExperienceTwo years customer service experience or experience in the insurance field/social work/medical fields(Above requirements can be met by the equivalent combination of education and experience). Licensure/CertificationThe American Heart Association’s Family and Friends CPR and/or Basic Life Support (BLS) training or retraining is required during orientation prior to providing patient care  CPR course and training must be maintained for the duration of employment ScheduleM-F Days Work ShiftDay Position TypeRegular Full Time
