ChristianaCare Primary Care is currently seeking a Full-Time Registered Nurse for our Primary Care location in Aston PAThe nurse will provide quality, safe, evidenced-based nursing care to achieve optimum outcomes both independently and as a valued team member of the practice. Plus, take advantage of free financial coaching, counseling, and budgeting support.Duties / Responsibilities:Demonstrate Knowledge & skills necessary to provide care appropriate to pediatric, adolescent,adult and geriatric patients virtually or on-site within the practice, including knowledge of growthdevelopment, the ability to obtain and interpret information to identify patient needs and providecare neededParticipates in a relationship-based care approach to care deliveryPerforms Nurse Visits such as Annual Wellness Visits, Advance Care Planning and TransitionalCare Management calls and disease managementPerforms point of care tests, EKG’s and other tests as directed by cliniciansTriages walk-in patients and ensures appropriate dispositionAnswers and responds to telephone messages under the direction of the providerAssists providers in examinations or proceduresFunctions as a liaison between patients, physicians and staff to provide accurate communicationCollaborates with team members to provide a smooth, organized and efficient flow of patientsResponds to patient crises situationsAssures adequate coverage within unit when neededProvides patient education and wellness counselingPerforms medication historyPerforms other related duties as requiredQualifications:A graduate of a state approved or National League of Nursing accredited School of NursingRN license or eligible for licensure in the state Delaware, Maryland or Pennsylvania & New Jersey iflocation of employmentTwo years’ experience as a RN, physician office experience preferredBSN preferred; ADN considered with agreement of completion of BSN in 3 yearsAbout ChristianaCareChristianaCare is rated by Forbes as the 2nd best health system for diversity and inclusion, and the 29th best health system to work for in the United States, and by IDG Computerworld as one of the nation’s Best Places to Work in IT.
