PRN Case Manager (RN)

The mountains are calling!!** **We're looking for a PRN Case Manager RN to join our Case Management team! ** We will hire a Registered Nurse or MSW for hospital discharge planners onsite at Valley View Hospital. Case Management and Epic experience a plus! _**Previous Case Management experience strongly preferred! **_ **GENERAL OBJECTIVES:** 1. Assists Valley View inpatients and outpatients and their families in the overall Management of their plan of care across the continuum. **QUALIFICATIONS:** Education: * BSN RN preferred. * Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program * State licensure as a Registered Nurse (RN). * 1 year of nursing (RN) experience Experience: * Case management experience preferred. * Computer experience (word processing, spreadsheet) and familiarity with Meditech * We are interested in every qualified candidate who is eligible to work in the United States.
