Position Summary:
The Ambulatory Care/ Oncology/Hospital Pharmacist is responsible for all aspects of ambulatory care/oncology and hospital pharmacy.  The pharmacist is responsible for dispensing prescription and non-prescription medications to patients in the inpatient, outpatient and emergency department settings. The Pharmacist provides patients with information about their medication regimen and helps ensure understanding of the medications. The Pharmacist must have scientific aptitude, excellent communication skills, and be detail oriented. The Pharmacist advises physicians and other health care practitioners on the selection, dosages, interactions, and potential side effects of medications. The Pharmacist will perform duties in the Disease and Medication Management clinic, Oncology and Outpatient Infusion and hospital pharmacy. While keeping current on regulatory compliance and tracking of quality assurance assessments to promote quality/continuity of care. The Pharmacists monitor the health and progress of patients in response to drug therapies. Pharmacists will also help patients manage chronic conditions such as anticoagulation, asthma, COPD, heart failure, hypertension, and medication management per collaborative practice agreement in conjunction with an interdisciplinary team.
Schedule: Full-Time, 40 hours/week (1.0 FTE)
Minimum Qualifications: 
Doctor of Pharmacy (Pharm D) or be a graduate of an accredited pharmacy program prior to the institution of the Pharm D. 
Incumbents must pass the North American Pharmacist Licensure Exam and be licensed to practice pharmacy in the state of Montana. 
Previous ambulatory care and/or hospital pharmacy experience is a plus. 
Computer experience required.  
Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) must be obtained within 6 months of hiring and Immunization certification are required. 
Responsibilities: 
Disease and Medication Management clinic: Managing patients under collaborative practice agreement
Prepare medications and supplies for dispensing including prepacking bulk medications; filling PIXIS machines; compounding ointments, creams, oral solutions and other medications; compounding total parenteral nutrition solutions; compounding intravenous admixtures; checking continuous unit dose medications; control and audit narcotics/stock substance; compounding and handling neo-plastic agents 
Verify orders and order entry. Ensure orders meet the patient’s plan of care.
The hospital pharmacist will meet the pharmaceutical care needs of the following patients:  neonatal, pediatric, adolescent, adult, and geriatric. 
Maintains professional competency by completing annual competencies as required by licensure and professional organizations.
Develops and maintains a patient pharmacy profile.  Monitors for drug interactions.
Checks medication carts filled by techs, assists nurses with questions, and monitors medication administration. 
Serves on assigned hospital committees.
Assists with coordination of pharmacy work-load.
Will oversee pharmacy technicians on duty.
