Job Description

The RN Care Coordinator will manage and facilitate treatment plans for care coordinated across our enterprise. This role will support patients at our Elbow Lake and Morris sites and also take new referrals. As a Care Coordinator you will assess patients’ needs and collaborate with a multidisciplinary team to develop a comprehensive plan of care. With the plan of care, you will work with the patient’s health care providers and coordinate the treatment and community resources available to the patient across the continuum of care. You will monitor patient goals, encouraging them to follow their doctor’s recommendations, and documenting improvements or setbacks in their condition. Duties include but are not limited to: Conduct patient assessments and reassessments, developing individualized care plans in collaboration with patients, families, providers, and the multidisciplinary care team. Coordinate patient care services, community resources, and pre-visit planning to ensure seamless, high-quality care delivery. Manage patient registries, monitor quality and satisfaction outcomes, and identify opportunities for program and process improvement. Develop and maintain patient and community education materials, support outreach initiatives, and assist with marketing care coordination services. Prepare and present program performance data and key outcomes to leadership and stakeholders. Support team collaboration through meeting facilitation, staff training and development, certification activities, and other assigned responsibilities. Hours/Shifts 80 hours biweekly, day shifts, Monday to Friday. This role will work out of our Elbow Lake and Morris locations, with occasional coverage in Fergus Falls when needed. Compensation This is a salaried-exempt role with an hourly wage range equivalent to $40.45-$54.37/hour. Please note that final compensation will be determined based on experience, qualifications and internal equity considerations. Education/Special Skills: Registered Nurse with current licensure in Minnesota and a minimum of 2-5 years of experience. Have proven working relationships with primary care providers, experience with patient/staff education, experience managing resources either directly or within the implementation of a new initiative. Strong data analysis, involving interpretation of clinical results to develop program improvement projects required. Have strong attention to detail and follow-up capacity, excellent organizational skills, and ability to multi-task. Background in chronic disease management preferred. Successful completion of the pre-employment health screening process required. Be able to attend the work site per schedule. Be willing to obtain Case Management certification when eligible
