Job Description

This position is 64 hours per biweekly pay period and includes daytime hours scheduled between 8:00 a.m.- 4:30 p.m. Monday - Friday. Understands and uses AIDET principles to promote an environment of dedication to providing excellent customer services to our patients and their families. 2. Identifies, develops, coordinates, and delivers nutritional counseling for inpatients, outpatients, and community events/education programs. 3. Seek to understand and meet the needs of customers (patients, families, vendors, co-workers, etc.) through respectful, courteous and culturally sensitive interactions. 4. Assess, monitor, and implement treatment plans for patients and provide ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care as required. Document assessments, interventions, and plans including recommendations, goals, progress notes, and discharge summaries. 5. Works with Director of Clinic Operations and other departments to ensure patients' dietary needs are met. 6. Develops and evaluates educational materials related to services provided, using pertinent and current research to provide client diet education. 7. Involved in the creation and implementation of wellness and health improvement programs. Will be a reputable resource for nutrition and wellness programs at Astera Health and the community it serves. 8. Develops, implements and participates in quality improvement initiatives. 9. Develops, reviews and updates department policies for the clinical aspect of patient care in accordance with The Joint Commission and facility standards. 10. Serves as an educator in the diabetes education and nutrition education department and assists in planning and goal setting. 11. Collaborates with the RN Diabetic Educator to maintain diabetes resource program accreditation through American Diabetes Association as necessary. Plan and execute annual diabetes advisory committee meeting. Assist with patient statistics needed for American Diabetes Association accreditation. 12. Collaborates with physicians and other health care providers to plan and implement patient nutrition care needs. Provide clinical resources and consultation to staff and physicians. **Core Competencies:** 1. Annual Mosby's education. 2. Minimum 15 CEU hours in diabetes specific topics annually. 3. Strong oral and written communication skills. 4. Demonstrates high degree of commitment to customer service excellence. 5. Strong computer skills, knowledgeable in Microsoft Office. 1. **** Ability to effectively operate standard medical equipment, electronic medical record software, and standard office equipment and software 1. Maintains confidentiality of information and privacy compliance. **Physical Requirements:** Medium Duty: exerting 20 to 50 pounds of force occasionally or 10 to 15 pounds of force frequently or greater than negligible up to 10 pounds of force constantly to move objects (based on U.S. Department of Labor data). The following physical activities are required to perform this job: Occasionally (up to 33% of work time): Standing, sitting, lifting, pushing, pulling, kneeling, crouching, smelling, color vision.
