LCSW-C Position Summary The LCSW-C for Palliative Care provides specialized psychosocial support to hospitalized patients and their families receiving Palliative Care. This role is an integral part of the Palliative Care Team, offering emotional support, resource referrals, and guidance during complex medical decisions. This position documents Social Work assessment, family meetings, interactions, family dynamics, and other pertinent information in the medical record. The Social Worker assists patients with completing advance directives and/or aides in obtaining previously completed advance directives. Also assists with triage of new consults as needed. Education Graduate of a Master’s degree program in social work from a college or university approved by the Council of Social Work. Required License and/or Certification Maryland State Social Work license, at the Licensed Certified Social Worker – Clinical (LCSW-C) level. Experience Knowledge of community resources and agencies is preferred. Proficient computer skills - Epic EHR experience is preferred. Knowledge of HIPAA and confidentiality, and patient rights and laws. 1 year of experience in a healthcare setting and/or experience with crisis intervention and supporting patients with psychosocial barriers to health is preferred. Mental Demands Must be able to organize effectively, work under constraints of time, pressure, accuracy and interruptions. Must possess good interpretive and communication skills, both oral and written. Must exhibit strong service excellence skills to include effective and timely communication, compassion, and motivation, when interacting with patients, families, the general public and other health professionals. Ability to work independently. Ability to multi-task with a high-level attention to detail. Ability to maintain composure in stressful situations. LCSW-C Work Schedule Primarily day shift Monday through Friday. Evening and weekend work as needed.
