We are looking for an experienced Primary Care Advanced Practice Provider to join our team where your expertise truly makes a difference. Education - APRN: Master’s or Doctoral degree from an accredited Nurse Practitioner program. - PA‑C (Physician Associate): Master’s degree from an ARC‑PA–accredited Physician Assistant program. Licensure, Certification, Registration - Unencumbered New Hampshire APRN or PA-C license. - National certification in relevant specialty (APRN) or NCCPA (PA-C). - Eligible for or current DEA license. Experience Prior primary care experience required. Responsibilities - Partner with patients to understand their health history, perform comprehensive examinations, and develop thoughtful, evidence-based diagnoses. - Use diagnostic tools effectively to guide care and ensure timely follow-up. - Manage a wide range of acute and chronic conditions with clinical expertise and judgment. - Prescribe and monitor medications safely, ensuring optimal patient outcomes. - Educate patients on preventive health and wellness to empower informed decisions. - Maintain productivity standards while delivering high-quality, patient-focused care. - Document care thoroughly and accurately, supporting continuity and quality of care.
