What You’ll Do
As a Home Health Physical Therapist, you’ll deliver high-quality physical therapy services while working alongside physicians, nurses, and fellow rehab professionals to create meaningful outcomes for every patient.
Your day-to-day will include:
Evaluating patients and developing individualized treatment plansDelivering hands-on therapy, therapeutic exercise, and functional trainingEducating and motivating patients to reach their mobility and recovery goalsUtilizing a wide range of treatment modalities (manual therapy, hydrotherapy, ultrasound, traction, and more)Monitoring progress and adapting treatment plans for optimal resultsCollaborating on discharge planning and designing home exercise programsSupervising Rehab Aides and supporting staff developmentMaintaining accurate documentation and ensuring patient confidentialityPromoting a safe, clean, and compliant treatment environment
What We’re Looking For
Active Colorado Physical Therapy License (required)BLS certification (required)Previous experience preferred—but motivated new grads are welcome to applyStrong clinical skills and a patient-centered mindsetExcellent communication and documentation abilitiesCommitment to quality care, teamwork, and professional growth
Physical Requirements
Ability to stand, walk, bend, stoop, squat, and assist patients throughout the dayMedium physical demand levelLift up to 50 lbs occasionally and 20 lbs frequentlyFull range of motion and manual dexterity to operate equipment and technology
