RN-Hematology/Oncology Coordinator-Hematology Oncology (LCCC)

Job Objective: Works collaboratively as an active member of the inter-professional team to provide therapeutic patient and family centered care in the Hematology/Oncology patient population in the Cancer Care Program. Collaborates in the development, execution and evaluation of the multidisciplinary plan of care. Establishes goals and strategies for meeting the care coordination, discharge, and/or continuing care needs across settings with the patient, family, and/or other care providers.
