What You’ll Do As a Physical Therapist, you will play a vital role in delivering comprehensive, patient-centered care. Reporting to the Rehabilitation Team Lead, you will: Perform comprehensive evaluations, assessments, and diagnoses Develop and implement individualized care plans tailored to each patient Provide hands-on therapeutic interventions in a home care setting Educate patients and families to support long-term health and recovery Collaborate with a multidisciplinary team, including specialty programs such as: Parkinson’s Disease care COVID-19 recovery Post-acute stroke rehabilitation Respond to changing patient needs with clinical judgment and adaptability Why Join Us? Flexible scheduling Opportunities to work in specialty care programs Ongoing professional development, including: Continuing education Clinical in-services Care management groups Supportive, team-oriented environment focused on clinical excellence and growth What You’ll Bring Education Bachelor’s degree or higher from an accredited Physical Therapy program Experience At least 1 year of experience in acute care, rehabilitation, or a related setting Home care experience preferred, but not required Licensure & Certification Active Massachusetts Physical Therapist (PT) license BLS certification (required within 30 days of hire) Key Skills & Competencies Strong clinical assessment and patient care skills Excellent communication and interpersonal abilities Proven critical thinking and problem-solving skills Ability to prioritize and manage workload in a dynamic environment Solid organizational and documentation skills Basic computer proficiency Commitment to maintaining patient confidentiality Ability to read, write, and communicate effectively in English Dedication to service excellence and patient satisfaction
