Job Description

The Patient Safety Specialist is a highly motivated clinical expert responsible for promoting patient safety and minimizing risk of harm across the organization. Utilizing advanced clinical knowledge and critical thinking skills, the Specialist conducts incident investigations, analyzes patient safety events, and identifies opportunities to improve systems, policies, and practices to prevent future adverse events. This role independently reviews high-volume, time-sensitive cases, performs data analysis, literature reviews, and facilitates departmental Root Cause Analyses (RCA) and Proactive Risk Assessments. The Specialist collaborates with teams across the organization through rounding, educational presentations, participating in risk assessments, and culture of safety surveys to identify system improvements. Serving as a departmental and organizational consultant, the Patient Safety Specialist administers the event reporting system, provides training, collects and analyzes safety data, and supports process improvement initiatives to drive meaningful change and enhance patient outcomes. **Qualifications** : * Associate's degree in Healthcare Administration, Nursing, Nursing Informatics, Business, Science, Information Technology, Health Information Management, or related field preferred. * Two (2) years of progressive experience in workplace safety, working directly with online safety-event reporting systems, or facilitating quality improvement initiatives in a healthcare setting preferred. * Certified Professional in Patient Safety (CPPS) certificate required within two (2) years of hire. **Primary Duties and Responsibilities:** * Administer and oversee the organizational event reporting system, ensuring timely reporting, investigation, and closure of incidents; train staff, maintain accurate data, and generate reports to support patient safety monitoring. * Review and analyze patient safety events, near misses, and adverse occurrences, including high-volume, time-sensitive case reviews, literature reviews, and data analysis to identify trends and areas for improvement. * Perform incident investigations and participate in risk assessments to identify potential or actual risks of harm to patients and recommend system improvements to prevent future adverse events. * Lead and facilitate Root Cause Analyses (RCA) and Failure Mode and Effects Analyses (FMEA) in accordance with accreditation and regulatory standards, and coordinate implementation of associated action plans. * Maintain and abstract data from the event management system to assist in tracking, trending, and reporting information relevant to quality, safety, and performance improvement committees. * Analyze patient safety data and metrics to evaluate the effectiveness of interventions and recommend strategies to reduce risk and improve patient outcomes. * Serve as a resource for departmental and organizational process improvement initiatives, utilizing performance enhancement tools, clinical expertise, and data insights to guide change projects. * Provide consultation to departments and leadership regarding patient safety policies, practices, and event reporting processes. * Facilitate educational presentations, departmental rounding, and culture of safety surveys to engage staff and foster a proactive safety culture. * Ensure compliance with internal policies, external regulatory requirements, and accreditation standards related to patient safety. * Partners with performance improvement program managers, specialists, and infection prevention team members to support organizational goals and maintain continuity of departmental operations. **Compensation:** ** $31.55-$43.49 DOE** **Benefits:** * Health Insurance * Vision Insurance-- * Dental Insurance * Flexible Spending Account (FSA and Health Savings Account (HSA) Options * 401K Plan with Roth Options * 401K 3% employer match and a 2% base non-elective employer contribution * Eligibility for company-paid benefits such as life insurance and long-term disability, subject to applicable waiting periods * Option to elect Supplemental employee, spouse, and child life insurance * Paid Time Off (PTO) * Access to the Employee Assistance Program * Discounts: available for meals in the cafeteria and over-the-counter pharmacy items * Exercise/Fitness Facility: access to the Rehab Therapy Fitness Center for the employee and dependents
