Help Ensure Safe and Accurate Medication Care We are seeking a detail-oriented and compassionate Medication History Specialist to join our Pharmacy team. In this role, you will work directly with patients and their families to gather accurate medication histories that help ensure safe and effective care throughout the hospital stay. Your work plays a critical role in the medication reconciliation process, helping healthcare providers make informed decisions and reducing the risk of medication errors.
