Job Description
The Transplant Coordinator is responsible for collaborating with the health care team to develop theory-based patient care for those patients undergoing solid organ transplantation (heart, liver, lung, kidney, pancreas or multi-organ) from referral through long term post-transplant follow-up. Responsible for ensuring the coordination of the clinical aspects of transplant patient and living donor care permissible within their license and acts as a liaison between a kidney transplant program and dialysis facilities, as applicable. Promotes health and the prevention of injury and disease, serves as an advocate and educator, mentors, delegates, and provides feedback to their support team. Responsible for performing job duties in accordance with the mission, vision and values of Tampa General Hospital.
QUALIFICATIONS:
Required: Bachelor's Degree Nursing License Registered Nurse Work Experience and Additional Information Two (2) years of critical care experience. Experience in nursing and transplantation and a global transplant nursing perspective with a focus on long-term outcomes. Technical Knowledge, Skills, and Abilities · Expertise in transplantation and end-stage organ disease management. · Evidenced-based practice related to current research, guidelines, and evidence related to transplant and organ disease management. · Diagnostic skills to proficiently initiate and interpret diagnostic tests relevant to transplant and organ disease. · Electronic Medical Record (EMR) documentation. · Postoperative Monitoring · Quality Assurance and Process Improvement · Patient Education and Consultation · Collaboration and Conflict Management · Dependability and Professional Conduct · Resource Utilization and Innovative Problem-Solving
