Nurses are the backbone of Valley View, combining compassionate care with clinical expertise. Opportunity for an RN to join our Women's Health Clinic team as a full-time Care Coordinator. As the Care Coordinator you will work with the team to support patients with chronic conditions and/or complex needs. You will develop, manage, and update highly individualized care plans for individuals at-risk for poor outcomes and more! Click on the following link to learn more about Women's Health Clinic:Women's Health Services Glenwood Springs, Basalt, Eagle & Silt QUALIFICATIONS: Education: * Current Colorado RN license or ability to obtain. * BLS Certification through the American Heart Association Skills: * Effective communication - both verbal and written * Excellent interpersonal skills * Ability to affect change. * Ability to perform critical analysis. * Self-Directed * Positive Attitude Experience: * Minimum 2 years RN work experience. * Three to four years RN experience in the area of specialty is preferred. * Experience in Case Management a plus.
