Job Description
Title Pharmacist # Summary Responsible for dispensing medications for parenteral and enteral use, monitoring patient drug therapies, responding to emergency situations and providing accurate drug information to patients and health care providers.# Supervising pharmacy technicians. In the absence of a pharmacy director, assume the essential responsibilities and perform the duties of the pharmacy director as needed.# This position does provide direct patient care. # Compensation We are committed to providing compensation information for all open positions. The wage range for this position is $63.20#$89.23#per hour. Compensation is determined based on experience, education, and internal equity considerations. This reflects the base pay for the role. Key Accountabilities Maintain and monitor patient electronic medication profile according to established policies, procedures, and protocols.# Prepare and dispense medications per physician request according to established policies, procedures, and protocols.# Ensures safe, appropriate and cost-effective medication therapies according to established policies, and protocols.# Contributes to the quality and effective operation of the pharmacy department.# Maintains competence required for pharmacist.# Responds to emergency situations and prepares medications for administration per physician order. Accurately maintains dispensing and inventory records complying with state and federal regulations.# Handles hazardous pharmaceuticals in accordance with hospital protocol, including properly disposing of hazardous wastes, ensuring hazardous waste disposal containers are closed, and recognizing any hazardous waste spills or emergencies and contacting the hospital#s hazardous waste coordinator accordingly.# Fulfill employee prescription processing according to established policies, procedures, and protocols.# #Promotes appropriate antimicrobial stewardship practices through daily review of:# Review antimicrobial prescribing for appropriate med, dose, route, duration, and indication.# Adjust doses in cases of organ dysfunction.# Optimize doses in cases of organisms with reduced susceptibility.# Convert parenteral doses to oral when appropriate according to NH+C#s #Automatic Conversion of IV to Oral Medications# policy / procedure.# Identify duplicative therapy.# Recommend / implement de-escalation of therapy.# Monitor adverse events associated with antimicrobials# Performs other duties as assigned.# Required Qualifications# B.S. or Pharm.D. from an ACPE school of pharmacy# Registered as a licensed pharmacist with the Minnesota Board of Pharmacy# Must be able to speak, read, and write English proficiently to ensure safe and effective communication in a healthcare environment# Crisis Prevention Institute Foundations Course Certification or obtain within six months of hire# Preferred Qualifications## One or more years of experience as a hospital pharmacist# Experience responding to emergency situations# Ability to provide drug information using departmental resources or reputable internet sites in a clear and concise manner# Pharmacokinetic monitoring skills# Proficient preparation of compounded sterile preparations including IV chemotherapy according to established policies, procedures and protocols.# Good organizational skills# Schedule Varying Days/Shifts; 7:00 a.m. # 3:30 p.m. and 9:30 a.m. # 6:00 p.m. Weekend opportunities are available but not required and may be worked based on employee preference Benefits Earned Safe and Sick Time (ESST)#accrued on first paycheck Pension Plan (PERA) with 7.5% employer match Optional 457(b) Retirement Plans Loan Forgiveness Eligibility # PSLF#+#MN state programs for qualifying roles Employer-Provided Certifications +#Continuing Education Additional perks: Free On-Site Parking, Qualified Bilingual Staff Program, Employee Assistance Program, and discounts on cafeteria, scrubs, and OTC pharmacy items # About Us Northfield Hospital + Clinics#is a trusted, independent healthcare system with more than 800 employees.
