The Medication Safety Clinical Pharmacy Specialist is responsible for the development, implementation, coordination, and oversight of pharmacy-related medication safety programs throughout ChristianaCare, with a primary focus on acute care settings. Working closely with Pharmacy Leadership, this individual will lead efforts to optimize medication-use safety, regulatory compliance, quality improvement, and operational performance across the health system.Key ResponsibilitiesDevelop, implement, and maintain medication safety policies, procedures, and best practices in collaboration with Pharmacy Leadership.Provide oversight of medication-related order sets and clinical decision support tools within computerized provider order entry (CPOE) systems to ensure medication safety standards are met.Ensure alignment with nationally recognized medication safety standards and best practices, including guidance from organizations such as ASHP and ISMP.Conduct audits, assessments, and inspections to identify medication safety risks and opportunities for improvement.Investigate, analyze, and report medication errors, near misses, and adverse drug events.Lead departmental medication safety initiatives and projects, monitoring outcomes and reporting findings to Pharmacy Leadership.Serve as a medication safety expert during regulatory surveys and accreditation activities.Provide proactive consultation and reporting on medication safety issues throughout the health system.Represent Pharmacy Services on multidisciplinary committees and organizational initiatives as appropriate.Lead the Department of Pharmacy's Medication Safety related committee(s)Collect, analyze, and interpret medication safety data from multiple sources to identify trends and drive improvement efforts.Develop and implement strategies that enhance operational efficiency while reducing the potential for medication-related errors.Foster a culture of safety through leadership, collaboration, and role modeling.Assist in the education and precepting of pharmacy residents and pharmacy students.Deliver accredited and non-accredited educational programs for pharmacists, providers, nurses, and other healthcare professionals.Maintain current knowledge of emerging medication safety trends, regulations, and industry standards.Collaborate effectively with colleagues and stakeholders while promoting a respectful and professional work environment.Adhere to all organizational and departmental safety standards and promptly report unsafe conditions or practices. Education and Experience Requirements:Doctor of Pharmacy (PharmD) degree from an ACPE-accredited School of Pharmacy.ASHP-accredited PGY1 Pharmacy Practice Residency or equivalent experience (minimum three years of acute care pharmacy practice).ASHP-accredited PGY2 Pharmacy Residency or equivalent experience (minimum three years of acute care pharmacy experience in addition to PGY1 residency or equivalent experience).Current pharmacist licensure in the State of Delaware or eligibility for Delaware pharmacist licensure through reciprocity. Preferred QualificationsBoard Certification (e.g., BCPS or other relevant specialty certification).
