WakeMed Physician Practices is seeking an Ambulatory Care Nurse I (ACN) to join our growing Urology team in North Raleigh. This role offers the opportunity to build meaningful relationships with patients while providing expert clinical support and care coordination throughout their healthcare journey. As an Ambulatory Care Nurse, you will serve as a vital liaison between patients, providers, caregivers, and the interdisciplinary care team. You will play a key role in triaging patient needs, delivering education, coordinating care plans, and ensuring patients receive timely, high-quality care in the outpatient setting.
