JOB SUMMARY Working in conjunction with the Home Health Manager and RN, the CNA performs a variety of basic patient care activities in caring for the personal needs and comfort of patients within the framework of the Wyoming Nurse Practice ACT. Provides household management activities to enable patients in independence at home. Assists patient with self-administered medications and emotional supports. Requires a high school diploma or equivalent. Requires certification as a Home Health Aide within 6 months of hire. ESSENTIAL FUNCTIONS Assist with basic patient care and daily activities according to patient plan of care, including ambulation, dressing, activities of daily living, and patient safety. Provides service excellence utilizing AIDET. Observes patients and reports unusual conditions and/or reactions to RN/LPN. Maintains a clean and safe environment. Performs routine cleaning tasks. Documents according to policy. Demonstrates efficient time management and prioritization skills. Alerts staff when supplies are needed and replenishes stock. Participates in committees and PI teams in conjunction with strategic plan. May be required to work on other nursing units. Complies with the hospital#s Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures. Must be free from governmental sanctions involving health care and/or financial practices. May be required to set up Life Line units in patient#s homes. Performs other duties as assigned. JOB QUALIFICATIONS Education High school graduate or GED Certification Currently certified in the State of Wyoming. Certifications required See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy. Experience Previous CNA experience preferred.
