Homecare Resource Nurse Coordinator

Position HighlightsDepartment: Home Care Clinical AdminHours: 38 hours per weekShift: Shift 1 Work Schedule:  Days (must work one weekend day per month 8a to 4:30p and one holiday per year.)Job SummaryPerform triage to ensure Homecare patients receive necessary services. Assist patients with new acute concerns, assists management and collaborating departments with coordination of patient care. In addition, performs evaluations for complex/high acuity patients and/or assesses for any safety concerns for both staff and patients. Job ResponsibilitiesUnder Limited Supervision:Assists with triage and coordination of care rendered to patients and families by field staff.Provides oversight of daily schedule to ensure all patient needs are met.Coordinates and collaborates with scheduling department and Clinical Managers.Assesses complex cases to evaluate staff and patient safety and assure complex medical needs of patients can be met.Performs new admissions, resumptions of care and joint visits as identified by Clinical Program Director.Mentors/trains new staff in field to ensure technical skill set meets all standards. As assigned competencies staff on-site for wound vacs, drains, tubes, etc…. Special projects and other job related duties as assigned.Upholds both the Connecticut Nurse Practice Act and American Nurses Association Scope and Standards of professional practice. Adheres to all Core Values (Compassion, Pursue Excellence, Cooperation and Collaboration, Upholds Honesty, and Supports Innovation).Adheres to all Absolutes (Privacy and Confidentiality, Professional Appearance, and Responsibility and Commitment). Other job related duties as assigned. QualificationsMinimum Qualifications: A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut or a non RN licensed clinician such as an Occupational or Physical Therapist may also fill this position with an RN administrator on call.All candidates must be computer literate and able to work in multiple systems.
