Summary #### The Home & Supportive Care Certified Nursing Assistant (CNA) provides essential personal care and supportive services to patients in their homes or care facilities across both home health and hospice settings. Working under the direction and supervision of a Registered Nurse (RN), the CNA assists with activities of daily living (ADLs), monitors patient condition, and ensures safety, comfort, and dignity for patients facing chronic illness, recovery, or end-of-life care. This role is central to promoting quality of life and compassionate support in a primarily home-based setting. #### Essential Duties and Responsibilities #### The duties and responsibilities described below represent the general tasks performed on a daily basis. Additional duties may be assigned as needed to support the organization's vision, fulfill its mission, and uphold its values. #### Personal Care & Support (Per Established Care Plans) * #### Provide hands-on assistance with bathing, grooming, dressing, toileting, oral care, and other hygiene needs. * #### Assist with safe transfers, mobility, positioning, and ambulation, including the use of assistive devices. * #### Offer support with nutritional needs, including feeding assistance and hydration reminders. * #### Promote patient dignity, independence, and emotional comfort during all care tasks. * #### Provide a compassionate presence for hospice patients and their families, including at the end of life. #### Observation & Communication * #### Monitor and document changes in patient condition, behavior, or needs. * #### Report clinical changes or safety concerns promptly to the supervising nurse. * #### Support timely and thorough documentation of visits and care provided in accordance with organizational policies. #### Home Environment & Safety * #### Help maintain a clean, safe, and orderly environment for patient comfort. * #### Perform light housekeeping tasks related to patient care (e.g., linen changes, laundry, light cleaning). * #### Promote infection control and follow universal precautions at all times. #### End-of-Life Care (Hospice Specific) * #### Provide personal care and comfort measures to hospice patients in alignment with the care plan. * #### Respect the cultural and spiritual beliefs of patients and families. * #### Support families and caregivers with emotional presence and practical assistance. * #### Recognize signs of imminent death and notify the hospice nurse as needed. #### Working Conditions * #### Frequent travel to patient homes across the service area. * #### Exposure to diverse home environments. * #### In hospice settings, may involve providing care after death and supporting family members. #### Additional Responsibilities * #### Showcase strong time management skills, enabling the ability to make impactful judgment calls regarding patient care while balancing all other responsibilities and time commitments. * #### All Home & Supportive Care clinical staff function as integrated members of the broader service line team. They perform patient care encounters as assigned and may be temporarily reassigned to other areas within the service line in accordance with policy. * #### Attend required hospital-wide orientations, meetings, and in-services. * #### Demonstrate a commitment to flexible work scheduling when necessary to ensure patient care needs are met. #### #### Supervisory Responsibilities #### Reports To: RN Clinical Operations Manager, with day-to-day oversight from the Coordinator/Supervisor #### Supervises: None #### Responsibilities: None #### Qualifications #### To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. #### * #### *Education and Experience #### Minimum Required Education: High school diploma or equivalent. #### Preferred Education: Not specified. #### Experience: At least one (1) year of experience as a CNA in home health, hospice, or long-term care is preferred. #### Certificates, Licenses, and Registrations * #### Must hold current certification as a Certified Nurse Aide in Virginia. * #### Must hold, or obtain within 90 days of hire, and maintain American Heart Association Basic Life Support (BLS) certification. * #### Must hold and maintain a valid driver's license, automotive insurance, and an acceptable Motor Vehicle Record in compliance with organizational policy, and/or demonstrate a reliable method of transportation for work responsibilities.
